General Purpose
Responsible for serving as a mentor/advocate for students living in at-risk conditions to monitor school attendance, behavior, and academic engagement; setting goals focused on improving identified data points; updating families on progress; and, providing resources as needed for community- and school-based wrap-around services. He/she supports intervention(s) developed by the Child Assistance Team (ChAT). Additionally, he/she supports access to the Teen Centers or Family Center in Ogden School District.
Main Tasks and Responsibilities
- Monitor school data (attendance, behavior, academics) and help implement intervention(s)
- Meet consistently with student to set/discuss goals related to attendance, behavior, and/or academics
- Contact families consistently to update progress and share resources as needed
- Inform parents of available school and/or community programs to address risk factors
- Acts as a resource connector and maintains working knowledge of state, county, and local resources available
- Promote services within the community by establishing relationships with local schools, service providers, community groups, churches, age-specific programs, and other programming as necessary
- Organize homework nights or lunches for students to catch up on missing assignments
- Support OSD Youth Court
- Conduct community outreach, education, and advocacy in a culturally competent manner and ensure communication is reliable and scientifically accurate
- Participate as a member of related committees/team meetings (e.g. ChAT, CTMs for mentors and CHWs)
- Conduct home visits with other school personnel as needed
- Collect and compile information related to resources, community needs, organizational needs, etc. for program development, reporting, and evaluation; organize and report data as assigned for grants or program requirements
- Collaborate with related community partners as assigned or in relation to providing services to students/families
- Support Teen Centers and Family Center within Ogden School District to support students and/or families in accessing resources (e.g. food, mental health support, community resources)
Education and Experience
- Associate’s Degree or equivalent combination of education and experience
- Experience in: Social Work Services, Public Health Education, Community Health Education, Public Health, Lifestyle Management or a closely related field (preferred)
- One (1) year of related experience
- Successful completion of training in district and community programs
- Successful candidate will be expected to have or complete requirements for Community Health Worker credentials - completion of required components will be included in regular job responsibilities
- Successful candidate will complete Check and Connect training
- Experience working with community members and community-based organizations; building trusted relationships; interact professionally and express cultural awareness with culturally diverse individuals
- Excellent interpersonal and communication skills over the phone, text, and virtually; strong, efficient, detailed, organizational skills and able to work on several projects simultaneously; case management, resource/needs facilitation, outreach, and communication
- Ability to assess the needs of a diverse population; connect to a multi-racial/ethnic demographic; connect with resources on a federal, state, and county level
Working Conditions and Other Requirements
- Bilingual preferred
- Regular traveling throughout the district - must have a valid Utah Driver license and a good driving record
- Must be available for periodic evening and weekend presentations and activities
- The work environment will consist of office and community settings
- Must be able to work evening hours 2-3 nights per week
